Why do I not see any available supplements when trying to link them to a DDP1?

If requesting a change in service amounts, no supplements will show up due to the purpose of the form. In this case, only the supplement is required to be submitted as there is no requirement to link to a DDP1 form.

The guide for the DDP1 Supplement form can be found on OPWDD's website under Login and then CHOICES. This specific guide is used to give instructions on how to correctly use and fill out the form.

My DDP1 was returned and I can no longer link the supplement to it.

Upon returning the DDP1, the DDP1 supplement was also returned making it inactive. Because there is no "copy" functionality for the DDP1 Supplement, a new one will need to be created and then linked to the DDP1 for re-submission.

I am unable to submit my DDP1 Supplement.

If attempting to submit the DDP1 Supplement with a purpose of "Requesting a new service" you will be unable to submit the form as it must be saved and then linked to the DDP1 that is requesting the addition.

Please refer to the DDP1 Supplement Guide on the correct use according the the form's purpose.

I am unable to link a supplement to the DDP1 and the "Individual" field is blank.

This problem occurs when the forms were created without using the TABS ID within the TABS Lookup screen. In the future, it is strongly recommended to only conduct lookups using the TABS ID.

If this is the case, please also check your DDP1 to see if the "TABS ID" field within the form is blank. If so, a new DDP1 will have to be created using the TABS ID as the lookup. Without this field being filled in, you will not be able to link the supplement to the DDP1 as the system cannot match the TABS IDs.

When clicking on the supplement from the DDP1 form I am getting an "Access is Denied" error.

Users will see this error if the DDSOs within the DDP1 and Supplement do not match. Please note that if you intend to request that the form be returned in order to correct this, an entirely new DDP1 Supplement will need to be filled out in order to link to the DDP1.

I am unable to copy the DDP1 Supplement.

Users are unable to copy the supplement by design. This functionality is not available within the DDP1 Supplement form.
